Notice of Privacy Practices

This notice describes how medical information about your child may be used and disclosed, and how you can get access to that information. Please review it carefully. We are required by law to protect your child's health information and to give you this notice of our legal duties and privacy practices.
Our commitment
Enlight Autism Center is a health care provider, and we are required by federal law to keep your child's protected health information private. Protected health information, or PHI, is information that identifies your child and relates to their past, present, or future health, care, or payment for care. We are required to follow the terms of the notice currently in effect and to notify you following a breach of unsecured PHI.
How we may use and disclose health information
We may use and disclose your child's health information without your written authorization for the following purposes.
For treatment
We use health information to provide and coordinate your child's care. For example, our behavior analysts, speech-language pathologists, and occupational therapists share information with one another so the team can plan and deliver care together, and we may share information with other providers involved in your child's care, such as a physician or a school team, when you ask us to or when it is permitted.
For payment
We use and disclose health information to obtain payment for services, for example to verify coverage, obtain prior authorization, and submit claims to Medical Assistance, Medica, or another plan.
For health care operations
We use health information to run our practice, for example for quality review, training, scheduling, and administrative functions.
Other uses and disclosures permitted by law
Within the limits of the law, we may also use or disclose health information for public health activities, to report suspected abuse or neglect, for health oversight activities, in response to a court order or lawful request, to law enforcement as permitted, to avert a serious threat to health or safety, for workers' compensation where applicable, and to coroners, medical examiners, or funeral directors as the law allows.
Uses that require your written authorization
Most uses and disclosures of psychotherapy notes, uses and disclosures for marketing, and any sale of health information require your written authorization. Other uses not described in this notice will be made only with your written authorization, which you may revoke in writing at any time.
Your rights
- You may ask to inspect and receive a copy of your child's health information that we hold.
- You may ask us to correct health information you believe is incorrect or incomplete.
- You may request an accounting of certain disclosures we have made.
- You may request a restriction on how we use or share certain information, and we will accommodate reasonable requests where the law requires.
- You may ask us to contact you in a specific way or at a specific location.
- You may request a paper copy of this notice at any time, even if you agreed to receive it electronically.
- You have the right to be notified if a breach of your unsecured health information occurs.
To exercise any of these rights, contact our Privacy Officer using the information below. Some requests must be made in writing, and we will explain the process when you ask.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to this notice
We may change this notice and make the new terms apply to all health information we maintain. If we make a material change, we will post the updated notice and update the date at the top of this page.
